What is causing this? Is this limp mode? W204

So this is not the first time it is happening, but it becoming frequent. Does anyone have an idea what can be causing this? C250 4matic W204 '10 It dulls the engine drastically while driving, it does not change gear on time, when it changes and I release the gas and press it again, it starts from 1000rpm instead of picking up from the last gear.
