1980 280E - Looking for some pointers on where to start....

I have a 280E which has a few things going on that I want to get into good order. Not sure if its connected or not, but there is a whining sound that is there at idle, as well as driving, as well as a rough / lumpy idle. I've adjusted the screw to bring up the revs which has sorted out the slow thud and sometime stall, but still its a little rough. ANY idea appreciated, thank you
